Hospice RN Case Manager — Compassionate Patient Advocate
The Care Team
The Care Team Hospice is seeking a compassionate Hospice RN Case Manager to join our team in Barberton, OH. You will coordinate patient care, develop individualized plans of care, and collaborate with physicians, caregivers, and the interdisciplinary team to ensure comfort and quality of life for patients at end of life. We offer a supportive environment, on-site schedule Monday–Friday 8:00 AM–4:30 PM with on-call opportunities and competitive benefits, including tuition reimbursement and a #J-18808-Ljbffr The Care Team
