Hospice RN Case Manager - Compassionate Patient Advocate

Cadre Hospice is seeking an RN Case Manager to advocate for patients and coordinate personalized end-of-life care, collaborating with physicians, social workers, chaplains, aides, volunteers, and families. You’ll conduct comprehensive assessments, develop care plans, monitor progress, and educate patients and caregivers while supporting goals of care with dignity and compassion. Occasional on-call support and EMR documentation are part of the role. #J-18808-Ljbffr Cadre Hospice
