Overview This role involves providing comprehensive biopsychosocial/spiritual assessments, diagnosis, treatment, and advocacy for patients and their support systems. Overview This role involves providing comprehensive biopsychosocial/spiritual assessments, diagnosis, treatment, and advocacy for patients and their support systems. For CSWA: Under supervision, supports evidence-informed, person-centered care plans in collaboration with the interdisciplinary team, respecting patient self-determination and adhering to NASW standards. For LCSW: Independently conducts assessments, diagnoses, and treatment interventions, working across inpatient and outpatient settings to reduce health disparities and promote patient-centered care. Qualifications MSW with Oregon Clinical Social Work Associate License within 60 days for CSWA; MSW from accredited program and relevant licenses for LCSW. Minimum two years of clinical social work experience, with preferred experience in healthcare, hospice, or acute care. Skills & Responsibilities Conduct assessments, develop and implement care plans in collaboration with patients and teams. Work closely with various departments to support diverse patient populations. Respect patient autonomy, promote health equity, and adhere to professional ethics and standards.
