Autumn Lake Careers - We are seeking an experienced and motivated Admissions Director to join our skilled nursing and rehabilitation team! The Admissions Director will play a key role in managing the admissions process, building strong referral relationships, and helping ensure a smooth transition for residents and their families. Responsibilities of Admissions Director: Manage and coordinate the facility's admissions process from referral through admission Review referrals and work closely with clinical and administrative teams Communicate with hospitals, referral sources, residents, and families throughout the admissions process Build and maintain strong relationships with referral partners Coordinate admissions to help ensure a smooth transition into the facility Maintain accurate admission documentation and follow-up Work collaboratively with facility leadership to support census and occupancy goals Qualifications & Experience Requirements for Admissions Director: Previous Admissions Director experience in a skilled nursing facility is required Strong knowledge of the skilled nursing admissions and referral process Excellent communication, organization, and relationship-building skills Ability to manage multiple referrals and priorities in a fast-paced environment Strong attention to detail and follow-through Team-oriented with a positive and professional approach If you're an experienced Admissions Director looking for an opportunity to make an impact with a supportive team, we'd love to hear from you!
