Employee discounts Tuition Reimbursement You will join an experienced, hard-working team that values communication and teamwork. Responsibilities Complete admissions, social services evaluations, provide referrals, and communicate with residents and families. Assist guests with financial and legal matters, such as applying for pensions, and referring to lawyers or funeral homes for preplanning. Provide discharge planning services, including placement on waiting lists, arranging home care, and coordinating transfers. Arrange counseling services and psychiatric medication management as needed. Ensure accurate and timely documentation in MDS care plans. Qualifications Current social work licensure in the state Bachelor’s degree in social work required; master's preferred One year of supervised social work experience in healthcare is preferred About Ciena Healthcare Ciena Healthcare is Michigan’s largest provider of skilled nursing and rehabilitation services.
