Job Description
Physical Therapist (PT) Job Summary This position is focused on outpatient physical therapy, with responsibilities that may include floating to acute care as needed. The role involves delegating responsibilities to Physical Therapist Assistants and Aides, and delivering patient-centered care by utilizing assessment, planning, intervention, and evaluation in accordance with physical therapy standards, physician orders, and relevant policies and procedures. Essential Duties and Responsibilities
- Conducts evaluations to determine the patient's skeletal, neurological, respiratory, and cardiovascular status using formal and informal testing methods.
- Develops tailored treatment plans based on assessment data and the patient's tolerance for treatment.
- Creates and submits a written treatment plan to be included as part of the authorized plan of treatment, with updates as necessary to reflect changes in patient status.
- Designs a comprehensive treatment schedule that specifies frequency and duration, discussing it with the patient, family, or caregiver.
- Educates patients, families, and caregivers about their conditions, treatment options, prognoses, and the rationale for prescribed interventions.
- Supervises Physical Therapist Assistants, Physical Therapy Aides, Home Health Aides, and students in compliance with regulatory standards.
- Recommends adaptive equipment based on patient assessments, facilitating orders and securing equipment as needed.
- Documents patient responses to treatment and any changes in condition according to documentation policies and regulatory agencies.
- Completes daily documentation necessary for billing in alignment with organizational policies.
- Demonstrates treatment interventions and competently facilitates patient care to restore optimal functioning.
- Promotes patient education through effective teaching methods in home exercise programs.
- Maintains effective communication with all internal and external parties to enhance the quality of patient care.
- Engages in staff development through education and training, employing relevant teaching methods.
- Participates in program development, team conferences, and interdisciplinary meetings as appropriate.
- Advocates for the profession of physical therapy through community education initiatives.
- Acts as a case manager for assigned cases.
- Demonstrates commitment to organizational values through exemplary customer service and teamwork.
- Maintains confidentiality of all patient and organizational information in accordance with policies and procedures.
- Engages in further learning opportunities as required.
- Demonstrates effective problem solving, organizational skills, and initiative.
- Complies with Occupational Safety and Health Administration (OSHA) requirements annually as outlined in the Exposure Control Plan.
- Adheres to protocols relating to communicable diseases and personal protective equipment use.
- Ability to navigate stressful situations, including those involving public contact.
- Able to wear necessary personal protective equipment as required.
- Physical mobility to move freely throughout the facility.
- Visually and audibly functional with or without devices.
- Capable of sitting, standing, and walking for extended periods, as well as frequent postural changes.
- Willingness to manage multiple tasks in a fast-paced environment while maintaining regular attendance.
- Overall dexterity is required, including the ability to handle, reach, grasp, and feel.
- Must possess a Bachelor's Degree in Physical Therapy or a related field.
- State licensure as a Physical Therapist is required.
- BLS - Basic Life Support certification preferred within one month of hire.
- Completion of orientation and new hire competency requirements.
- Fingerprinting is mandatory; the cost will be covered by the organization.
