Case Coordinator- Administrative Case Manager

Responsibilities As the Administrative Assistant of Blended Case Management, you will: Assist with the overall administrative management of the BCM program (maintaining referrals, opening individuals in the service, assisting with running reports, verifying billing, etc.) Lead billing review process for county funding (completing and submitting monthly invoice, submitting and maintaining authorizations) Assist with the auditing medical records for compliance and documentation requirements Coordinate care for clients with mental health, medical, education, placement and related areas, while maintaining a small caseload (approximately 10hrs per week) Coordinate, monitor and review the provision of services to ensure clients are receiving and participating in services on a continual basis. Link client to appropriate resources Maintain direct contact via telehealth and some face to face with caseload and parent/guardian per state regulations Ensure timely and accurate completion of related documentation Assist other community service programs with administrative responsibilities on an as needed basis Offer Interaction with all levels of care with ability to work independently and also part of a case management team Flexible Schedule (mostly during the hours of 9am-6pm, with some evening work needed due to family availability) Benefit eligibility after 30 days of employment (1st day of the 1st pay period after 30 days) Pay rate of $19 - $21.66 an hour, dependent upon experience.
