The Case Manager is responsible for providing comprehensive support and oversight to clients by coordinating services, advocating for resources, and ensuring individualized care plans are implemented effectively. This role involves assessing client needs, developing action plans, monitoring progress, and collaborating with multidisciplinary teams to promote overall well-being and stability.
· Conduct initial intake evaluations for incoming clients referred from the shelter system, including social service assessments, psychosocial evaluations, development of independent living plans, and appropriate referrals to supportive services.
· Assist clients in obtaining identification documents, accessing public benefits, and securing other applicable income sources by providing guidance, support, and advocacy throughout the application process.
· Maintain accurate and confidential client case records for the assigned caseload, ensuring timely updates that reflect service delivery, client progress, and all relevant documentation requirements.
