Key Responsibilities

• Conduct thorough patient evaluations, including reviewing medical histories and performing specialized assessments to inform treatment plans.
• Communicate evaluation findings and treatment strategies to patients and their families, incorporating their input into goal-setting to ensure alignment with patient needs.
• Implement and adjust individualized treatment protocols based on patient progress and response to therapy, demonstrating adaptability to evolving healthcare demands.
• Collaborate with multidisciplinary healthcare teams to optimize patient outcomes, documenting any changes in patient conditions that affect the care plan.
• Manage the discharge planning process, preparing patients for follow-up care while ensuring accurate documentation of all treatment activities.

Qualifications

• Bachelor's Degree in Physical Therapy or related field.

Compensation and Benefits
