Pay: $62,000.00 - $72,000.00 per year Job Description Always Compassionate Home Care provides the highest quality of community home-based services, combined with exceptional compassion and innovation to enhance each patient's quality of life. The Intake Coordinator will directly educate a prospective patient through the intake process leading to enrollment with Always Compassionate Home Care. This person will work closely with referral sources, account managers, clinical team, and provider relations to ensure all tasks are efficiently completed associated with the enrollment process. The coordinator's direct job function would allow for a patient to receive the proper care they need and confirm all information about said patient has been recorded correctly. Qualifications Working knowledge of NYS DOH Health Care operations.
