We proudly serve patients and families across nine counties in Northeast Ohio, ensuring that no one faces life's final chapter alone.We are seeking a dedicated Registered Nurse (RN) Case Manager who is passionate about hospice care and committed to delivering exceptional clinical and emotional support to patients and families.ResponsibilitiesThe RN Case Manager will:Perform initial and ongoing comprehensive nursing assessments for hospice patients.Provide compassionate clinical care, including symptom management, treatments, and preventive nursing procedures.Deliver dietary counseling and patient/family education to support comfort and quality of life.Observe and promptly report significant changes in patient condition to the physician and interdisciplinary team.Collaborate with the IDG to develop, update, and implement individualized care plans.Teach, supervise, and participate in the ongoing training of nursing personnel.Maintain clinical documentation in compliance with Harmony Hospice standards, Medicare and HIPAA regulations.Participate in on-call rotation to ensure 24/7 patient support.Meet agency infection control, quality, and compliance standards.Attend required meetings, team conferences, and continuing education.Perform additional duties as assigned by the Director.ScheduleFive 8-hour days (Monday–Friday)Benefits401(k)Medical, Dental, and Vision InsuranceLife and Disability InsurancePaid Time OffMileage ReimbursementCell Phone reimbursement or work phoneSupportive interdisciplinary team environmentOpportunities for professional growthMeaningful, purpose driven work every dayRequirementsActive RN License (Required)Valid Driver's License and Auto Insurance (Required)Minimum 1 year of nursing and direct patient care experience (Preferred)Hospice experience ideal but not required—training availableCommitment to compassionate, patientcentered careWork LocationThis is a road-based position serving multiple counties within our service area on the West side of Cleveland and surrounding areasJoin the Harmony Hospice TeamIf you are a compassionate RN looking to make a meaningful impact while working with an exceptional, supportive team, we would love to speak with you.Apply today and help us bring harmony, dignity, and comfort to the patients and families we serve.Compensation NoticeThe pay range listed represents a general guideline for this role and does not guarantee the final offer.
