PRN RN Case Manager

The PRN RN Case Manager at Cleveland Clinic Hillcrest Hospital collaborates with inter-professional teams to facilitate effective transitional care coordination. Highlights Pay: $45.00/hour Location: Mayfield Hts, OH Facility: Hillcrest Hospital Schedule: Per Diem, Day shift What You'll Do Ensure compliance with standards of accrediting bodies and state mandates. Promote interprofessional synchronization throughout the acute episode. Complete effective handoffs to the next level of care or community. Proactively assess and mitigate risks to patients’ transitions of care. Work with patients and care partners to develop individualized transitional care plans. Participate and lead in patient rounding activities, huddles, and committee meetings. Requirements Education Graduate from an accredited school of professional nursing (Diploma, ADN, BSN program) required Experience 3 years of recent RN experience required 2 years of recent experience in utilization review, quality, or care management preferred Direct acute case management experience preferred Licenses & Certifications Current state licensure as a Registered Nurse (RN) Basic Life Support (BLS) Certification required Certification by an approved entity within one year of eligibility preferred (CTM, ACM, CCM, or AHA) About Cleveland Clinic Cleveland Clinic is a leading healthcare provider committed to delivering exceptional patient care.
