Care Coordinator

Schedule

This position requires 8-hour shifts from 8am to 4:30pm, with weekend and weekday availability.

Job Description

The Care Coordinator is responsible for coordinating care and services for assigned patients during an episode of illness or treatment. This role involves collaboration with the patient, family, physician, and patient care team to ensure optimal outcomes. Utilizing advanced clinical skills, the Care Coordinator facilitates the nursing process, including assessment, planning, intervention, and evaluation.

Responsibilities

• Coordinate patient care in collaboration with a multidisciplinary team, ensuring effective utilization of resources.
• Evaluate and analyze quality/utilization cases, admission/discharge criteria, and readmission metrics.
• Advocate for patients and families throughout the episode of care.
• Maintain communication among all providers and monitor services rendered.
• Participate in outlier reviews and initiate clinical pathways based on admission diagnosis.

Required Experience / Certifications / Licensure

• At least one year of recent Case Management experience in an acute care setting.
• Current CA Licensure.
• Current CPR recognition.

Assignment Duration

This is a 13-week contract position.
