Clinical Assessment RN - UAS-NY (In-Home)
VNS Health
VNS Health is seeking a qualified RN to conduct UAS-NY assessments and develop individual plans of care for members in their communities. The role involves in-home and telehealth evaluations, documenting needs, and coordinating with care teams across upstate and downstate New York. Strong communication, organization, and knowledge of Medicaid/Medicare regulations are essential.
