Key Responsibilities\n

• Conduct comprehensive psychosocial assessments utilizing interviews, observations, and review of medical records to identify emotional, social, and environmental factors influencing patient recovery.

• Provide individualized counseling sessions and support to patients and their families, helping to address adjustment issues, emotional challenges, and coping strategies as they navigate their rehabilitation journey.

• Work alongside a multidisciplinary rehabilitation team to develop personalized treatment plans that align with patients’ psychosocial needs and rehabilitation goals.

• Assist patients in accessing essential community resources such as support groups, vocational rehabilitation services, and housing assistance to facilitate smooth transitions post-rehabilitation.
\n
• Offer education to patients and their families about community resources and services available after rehabilitation to ensure ongoing support.
\n
• Engage in discharge planning and coordinate patient transitions back into their homes or other appropriate care settings, ensuring that needed services are in place.

Patient Population / Client Focus\n

In this role, you will be directly supporting patients within our hospice center who are facing a variety of challenges due to severe health conditions. This includes but is not limited to those recovering from strokes, spinal cord injuries, orthopedic surgeries, and neurological disorders. Your expertise will play a crucial role in improving their emotional and social well-being, fostering resilience, and enhancing their overall rehabilitation experience.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution is mandatory.

• Active state licensure as a Licensed Clinical Social Worker (LCSW) is required.

Compensation and Benefits\n
