Key Responsibilities\n

• Facilitate referrals to appropriate external agencies and services, ensuring clients receive comprehensive support across different aspects of their lives.

• Maintain meticulous documentation, including client records and progress reports, to track interventions and outcomes effectively.

Patient Population / Client Focus\n

This position focuses on a diverse range of clients, including children and families who may be experiencing distress due to issues like abuse, domestic violence, mental health challenges, or substance abuse. As a hospice social worker, you will play an integral role in providing emotional and practical support for patients with terminal illnesses, emphasizing dignity and quality of life for both patients and their families. Qualifications\n

• A Master's Degree in Social Work (MSW) or a related field is required.

• Current and unrestricted LCSW licensure in New Jersey.

Compensation and Benefits\n
