This position is fully remote, allowing you to work from the comfort of your own home while serving clients in need across Newark, New Jersey, and surrounding areas. You will engage with clients primarily through telehealth sessions, although in-person interactions may occur as needed, supporting flexibility and accessibility for our clients.

Key Responsibilities\n

• Conduct comprehensive assessments of clients’ conditions to identify their needs, strengths, and areas for improvement.

• Develop and implement individual support plans tailored to meet the unique circumstances and goals of each client.

• Engage in crisis intervention as necessary, providing immediate support to clients facing emotional distress or difficult situations.

• Monitor client progress and adjust treatment plans based on evolving needs and feedback, ensuring ongoing effectiveness.

• Facilitate referrals to community resources and services that may aid clients in their recovery journey and overall well-being.

• Maintain detailed and accurate documentation of client interactions, progress, and treatment outcomes to comply with professional standards and regulations.

• Participate in case conferences and collaborate with multidisciplinary teams to share insights and strategies for improved patient care.

Patient Population / Client Focus\n

In this role, you will primarily work with individuals and families dealing with significant emotional challenges related to end-of-life issues, grief, and mental health crises. Your efforts will support vulnerable populations, including children affected by parental substance abuse and adults navigating the emotional complexities of hospice care. Qualifications\n

• A Master’s degree in Social Work (MSW) from an accredited institution is mandatory.

Compensation and Benefits\n
