Key Responsibilities\n

• Engage with clients through phone consultations and virtual meetings to assess their needs and establish rapport.

• Conduct comprehensive assessments, gathering crucial information to design effective treatment plans tailored to individual client circumstances.

• Provide crisis intervention and support as needed, using established best practices to help clients navigate their immediate situations.

• Facilitate referrals to external agencies and resources, ensuring clients receive the comprehensive care they require.

• Participate actively in ongoing supervision and collaborative team meetings to discuss case management and share insights.

• Maintain meticulous records of client interactions and treatment plans, preparing reports for tracking progress and performance metrics.

• Identify and document concerns related to child abuse and other critical issues, ensuring timely intervention when necessary.

• Develop and implement strategies to involve clients in positive community activities that foster personal growth and recovery.

Patient Population / Client Focus\n

As a Licensed Clinical Social Worker at Revita Care, you will primarily work with diverse populations facing social and psychological challenges. Our clients include children, adults, and families who are dealing with neglect, physical and emotional abuse, mental health disorders, and the effects of substance abuse. A keen understanding of these issues will enable you to tailor your approach to meet each client’s unique needs.

Qualifications\n

• Master’s degree in social work, psychology, or a related mental health field.

• Current and valid license as a Clinical Social Worker (LCSW).
\n
• Proficiency in using technology, including Microsoft Office Suite (Word, Excel, Outlook, PowerPoint), for documentation and communication purposes.

• Demonstrated ability to communicate effectively with clients, actively listen, and provide empathetic responses to complex emotional situations.

• Experience in crisis intervention techniques and treatment planning.

• A valid driver’s license for any necessary community outreach or engagement activities.

Compensation and Benefits\n
