COMMUNITY RELATIONS DIRECTOR Position Summary The Home Health & Hospice Sales Representative is responsible for developing and maintaining referral relationships that drive growth for both Home & Care Home Health and Everwind Hospice. This individual serves as the primary community liaison, educating healthcare providers, facilities, and community organizations about our services while ensuring patients receive the most appropriate level of care. Success in this role is built on relationship development, exceptional customer service, clinical knowledge, and the ability to identify opportunities that improve patient outcomes while increasing referrals and admissions. Reports To Executive Director of Sales & Marketing Essential Responsibilities Business Development Develop and maintain referral relationships with physicians, hospitals, skilled nursing facilities, assisted living communities, case managers, discharge planners, and community partners. Execute weekly territory management plans to maximize referral growth. Conduct scheduled face-to-face sales calls each week. Develop new referral sources while strengthening existing partnerships. Represent the organization at networking events, health fairs, and community outreach activities. Coordinate educational presentations for referral partners and community organizations. Referral Management Identify patients appropriate for Home Health and Hospice services. Collaborate with clinical teams to ensure timely admissions. Communicate with referral sources throughout the referral and admission process. Monitor referral pipelines and provide follow-up on pending referrals. Assist referral partners in navigating eligibility requirements and payer guidelines. Customer Service Serve as the primary point of contact for assigned referral accounts. Resolve concerns quickly and professionally. Maintain regular communication with referral partners. Ensure an exceptional customer experience for patients, families, and referral sources. Marketing Promote Home & Care Home Health and Everwind Hospice services throughout the community. Coordinate physician offices, facility in-services, lunch-and-learns, and educational events. Assist with community marketing campaigns and social media initiatives. Participate in community outreach and volunteer opportunities. Collaboration Attend weekly sales meetings. Participate in interdisciplinary meetings as appropriate. Work closely with clinical leadership to identify patients appropriate for Home Health or Hospice. Share market intelligence and competitor activity with leadership. Performance Expectations The Sales Representative is expected to consistently meet or exceed performance goals including: New referral sources developed Weekly face-to-face sales calls Qualified referrals generated Home Health admissions Hospice admissions Referral conversion rate Customer service responsiveness Community outreach participation CRM documentation completed timely (once implemented). Referral partner satisfaction Qualifications Required Bachelor’s degree preferred; equivalent healthcare sales experience considered. Minimum of two years of healthcare sales, marketing, or business development experience preferred. Excellent communication and relationship-building skills. Strong organizational and time management abilities. Valid driver’s license with reliable transportation. Ability to travel throughout assigned territory. Must be able to successfully complete a fingerprint-based background check and pre-employment drug screening as a condition of employment, in accordance with applicable laws and regulations. Preferred Home Health or Hospice sales experience. Knowledge of Medicare, Medicaid, and commercial insurance. Existing healthcare referral relationships. CRM experience.
