About this Role:

DHCS is seeking an experienced population health management professional to fill the role of Chief, Population Health Management Division (PHMD) in Quality and Population Health Management (QPHM).

The Chief, PHMD leads strategy, policy, oversight, monitoring, and evaluation of population health management. The Chief, PHMD leads implementation of the population health management under CalAIM, which aims to help all members stay healthy via preventive and wellness services, identify and assess member risks to guide care management and care coordination needs, and identify and mitigate social drivers of health to reduce health care disparities.

The Chief, PHMD builds collaborative working relationships with the DHCS Directorate, Executive Staff, internal partners, and external stakeholders, including the California Health and Human Services Agency, managed care stakeholders, as well as other governmental entities at local, state, and federal levels.

The Chief, PHMD works under the general direction of the Chief Quality and Medical Officer and Deputy Director (DD), QPHM, and is a member of the DHCS Executive Staff.

This position is subject to the Department's Conflict of Interest Code and filing a Statement of Economic Interests (Form 700) upon assuming office, annually while in office, and upon leaving office.

• Demonstrated ability to perform high-level administrative, management, policy development, and implementation.
• Demonstrated ability to effectively communicate and lead population health efforts and coordinate with other areas within the Department, other departments, county partners, and external stakeholders.
• Demonstrated ability to lead the policy development, implementation, monitoring, and evaluation of population health management efforts across all programs spanning, health care, dental, mental health, substance use, home and community-based services, and social services.
• Demonstrated ability to use data and information to define and support teams in delivering on strategic and policy direction.
• Demonstrated ability to clearly synthesize problems and influence diverse stakeholders groups to address them.
• Demonstrated knowledge of Population Health Management, Enhanced Care Management system, Health Care Delivery Systems, Health Care Financing, Community Reinvestment, and Medi-Cal members.
• Demonstrated ability to lead teams in developing solutions to improve health outcomes and equity for the entire population while reducing costs.
• Demonstrated knowledge of healthcare product development.
• Demonstrated knowledge of administrative functions, including but not limited to budgets, contracts, and procurement.
