Part-Time Post-Acute Nurse Practitioner or Physician Assistant (Brookline, MA)
We are looking for a patient-centered Nurse Practitioner (NP) or Physician Assistant (PA) to provide post-acute care rounding in Brookline, MA. This position requires one weekend day per week and and incentive potential.
Position Details
- Schedule: Part-time, 1 day per weekend rounding in skilled nursing facilities
- Compensation: Fee-for-service model with an estimated $28,000 annual earning potential, plus eligibility for shared savings bonuses
- Setting: Post-acute care and skilled nursing facilities
Core Responsibilities
- Patient Care: Conduct comprehensive patient assessments, physical examinations, and clinical evaluations.
- Collaboration: Partner with an experienced multidisciplinary team and facility staff to develop tailored care plans.
- Relationship Building: Communicate closely with patients and families during key transitions in care.
