Key Responsibilities

• Deliver physical therapy treatments to residents under the supervision of a licensed Physical Therapist
• Assist in implementing individualized treatment plans in a skilled nursing facility setting
• Document patient progress and maintain accurate clinical records in accordance with facility standards
• Collaborate with the interdisciplinary care team to support resident rehabilitation goals

Required Qualifications

• Active Physical Therapy Assistant (PTA) license in the state of Illinois or eligibility for licensure
• Graduate of an accredited Physical Therapist Assistant program
• Current CPR/BLS certification

Preferred Experience / Skills

• Prior experience working in a skilled nursing facility or long-term care environment
• Familiarity with geriatric rehabilitation techniques and restorative care programs
