The Transition Services Manager coordinates services, best practices, and resources with all staff across selected secondary transition-related activities.

ESSENTIAL FUNCTIONS

• Facilitates transition planning process with families, classroom teams, and agencies, working towards a seamless transition from school to adult services for each graduate 
• Develops professional development content, in-service training materials, and parent support, education, and workshops in relation to improving post-school outcomes 
• Serves as embedded coach/mentor by bringing staff to mastery in evidence-based transition related model and practices, ensuring own attendance at transition related professional development to stay current in the field 
• Ensures the school career development model, IEP transition sections, resources, school employment services, and tools related to transition are up to date, meet PDE and ODP guidelines and regulations, and are in line to achieving desired outcomes for students 
• Oversees school career preparation services and team with an “employment first” approach and ensures services and billing are completed in accordance with funding regulations
• Serves as secondary transition lead person across all stakeholders and customers, including school districts, parents, , agencies, etc. 
• Provides coaching to all team members in a supportive and collaborative manner and reviews and implements coaching and feedback from others to improve performance 
• Reviews, submits, and manages timeline and other budgetary needs of department 
• Identifies, addresses, and manages employee performance discrepancies, disciplinary issues, and training deficiencies for department members 
• Implements performance management policies and positive management practices to result in high performing employees and teams 
• Completes other duties as assigned

PHYSICAL DEMANDS

The physical demands described are representative of those that must be met by an employee to successfully perform the essential functions of this job. The following positions or frequent movements are typical of the job: sitting, walking, standing, reaching, pushing, pulling, lifting, stooping, and crawling, often for extended periods of the workday. Assignment may require the ability to physically move students during possible physical interventions in order to maintain a safe learning environment.

The position requires that an individual be able to learn new tasks quickly and effectively. Job requirements may change frequently. The ability to understand and carry out detailed, involved, instructions is mandatory. The incumbent must be able to travel to multiple locations as needed, when needed to conduct organizational business. Subject to indoor conditions based on assignment.

EXPECTED SCHEDULE

This is a full-time position with a minimum 40-hour work week expectation. The Transition Services Manager is required to follow the school calendar, including an obligation to work the 205-day student calendar in addition to 13 days of professional in-service.

DESIRED EDUCATION AND EXPERIENCE

• Special Education Certification and/or pertinent experience with special education, secondary transition, and preparing students with disabilities for employment
• Supervisory experience preferred
• Strong collaboration and communication skills
• Customer service strength across a variety of stakeholders
• Successful completion of all pre-employment requirements
