SCAN is hiring a Biling Care Planner-MSSP in Los Angeles. The role provides long-term care management to culturally diverse older adults and caregivers, including in-home assessments, care planning, and coordination with community-based resources. The ideal candidate will have a BA in Social Work or related field, experience with the elderly, and bilingual English/Spanish proficiency. Travel across Los Angeles County is required; a valid driver’s license and reliable transport are expected. #J-18808-Ljbffr SCAN
