RN: In-Home Health Assessments for Managed Care
Activate Care
Activate Care is seeking a Registered Nurse to perform in-home health assessments for dually eligible members across Massachusetts. You will document findings in the health plan's system, then hand off to care management. Role emphasizes assessment and documentation, not ongoing care management. Must have MA RN licensure, driver’s license, and the ability to travel; familiarity with GuidingCare and HIPAA is preferred.
