What You'll Do

• Screen, evaluate, and develop treatment plans for patients with physical functioning disorders and wounds
• Administer therapeutic interventions, including manual techniques, exercises, modalities, and wound care
• Document patient progress, treatment outcomes, and discharge summaries in electronic records
• Communicate with patients, families, and healthcare team members regarding care plans and progress
• Support and supervise rehab team members, including assistants and aides
• Ensure compliance with all regulatory and clinical requirements and standards

What You Need

• Valid Physical Therapist license for Oklahoma
• CPR certification
• Minimum 2 years of relevant PT experience, preferably in LTC settings
• Strong clinical assessment and documentation skills

• Experience with wound care and complex patient cases
• Ability to work independently and as part of a multidisciplinary team
