Key Responsibilities\n

• Deliver compassionate and empathetic care to clients, actively listening to their concerns and needs.

• Conduct comprehensive assessments of clients' emotional and mental health, developing tailored treatment plans aimed at achieving their therapeutic goals.

• Facilitate individual and group therapy sessions, encouraging participation and fostering a supportive environment.

• Utilize evidence-based practices and methodologies to address various mental health conditions such as anxiety, depression, and grief.

• Continuously evaluate the effectiveness of treatment plans and modify as necessary to better serve clients.

• Stay informed on the latest developments within the mental health field to ensure best practices are being applied in treatment.

• Document all patient interactions accurately and timely, in compliance with HIPAA regulations, to ensure record integrity.

• Perform additional duties as assigned, including engaging in team meetings and professional development opportunities.

Patient Population / Client Focus\n

You will primarily work with adult patients in a hospice setting, providing emotional support and resources as they navigate end-of-life challenges. Our clients often require holistic and individualized approaches to addressing their mental health needs, especially during such a sensitive time. You will actively collaborate with other healthcare providers to ensure integrated care that encompasses both physical and mental health.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Current and valid LCSW license in California.
\n
• Minimum of 2 years of relevant clinical experience as an LCSW.

Compensation and Benefits\n
