Job Description

We are seeking a dedicated Hospice Registered Nurse with a minimum of one year of hospice experience. This role includes providing compassionate care to terminally ill patients in their homes or nursing facilities. The RN will work collaboratively in a small team environment focused on delivering high-quality care to patients with end-stage diseases such as cancer, heart disease, lung disease, and dementia.

Responsibilities

• Conduct 5-6 patient visits per day, ensuring comprehensive care in accordance with patient needs.
• Utilize EMR system EPIC for efficient charting and patient management.
• Coordinate with other healthcare professionals to deliver integrated care.
• Monitor patient conditions and adjust care plans as necessary to enhance quality of life.

Required Experience / Certifications / Licensure

• Minimum one year of hospice nursing experience.
• Active West Virginia RN license.
• Basic Life Support (BLS) certification from AHA.
