The physical requirements listed here represent what must be met by employees to successfully
perform the essential job functions of this position. 
Position has limited office environment work with frequent travel throughout the community.
The position routinely requires the ability to drive, travel by public transportation, speak, hear,
see, read, write, and speak in front of groups and the use of a computer, telephone and other general office equipment.

1. Drive to various locations and speak with group or individual about Key Family Care
LLC, its mission and services.
2. Build personal relationships with potential referral sources through on-going visitation.
This activity requires a friendly and professional attitude and a detailed knowledge of
Key Family Care LLC services and programs.
3. Be a liaison between Key Family Care LLC and referrals sources including physicians’
offices, discharge planners, assisted living and nursing homes and social service agencies
to facilitate the ease of making a referral to Key Family Care LLC.
4. Promote a positive professional agency image when speaking to groups or individuals
and through health fairs, community activities, and other events.
5. Identify additional channels for outreach and develop community contacts and make calls
to initiate a better understanding of Key Family Care LLC.
