Overview

Join our dynamic healthcare team as a Nurse Practitioner, where your expertise will directly impact patient outcomes and elevate the standard of care. This role offers an exciting opportunity to assist homebound patients in utilizing advanced skills in patient assessment, diagnosis, and treatment. You will travel to patients' homes to provide assessments and basic care for those who have chronic conditions. We help close the gap in quality primary care.

You will collaborate closely with the interdiciplinary team to deliver comprehensive, patient-centered care while adhering to regulatory standards and best practices. We are seeking a motivated, detail-oriented professional eager to make a meaningful difference in patients’ lives through innovative and compassionate care.

This is an 'on the road' position. Hourly rate is paid for travel between patients.

Duties

• Conduct thorough physical examinations and patient assessments, including vital signs, medical history review, and physiological evaluations.
• Prescribe medications and treatment plans in accordance with state regulations and collaborative practice agreements.
• Manage acute and chronic conditions across various specialties such as internal medicine, geriatrics, behavioral health, and emergency medicine.
• Perform procedures including suturing, IV insertion, catheterization, and dermal fillers when appropriate.
• Utilize electronic health record (EHR) systems for accurate medical documentation and medical records management.
• Collaborate with physicians, specialists, nurses, and other healthcare providers to develop coordinated care plans.
• Provide health coaching and patient education on disease prevention, medication adherence, nutrition, and lifestyle modifications.

Skills

• Extensive experience working with diverse patient populations including pediatrics, geriatrics, hospice & palliative medicine, and individuals with developmental disabilities.
• Proficiency in EMR/EHR systems such as Epic, Athenahealth or Cerner for documentation and clinical workflows.
• Skilled in performing diagnostic evaluations using laboratory experience, sonography, medical imaging, and specimen processing.
• Strong knowledge of pharmacology for medication administration including injections and drug interactions under DEA regulations.
• Experience working within collaborative practice agreements to optimize patient care delivery across outpatient clinics or inpatient units.
