Care Coordinator- Fresh Meadows

Spear Physical and Occupational Therapy is seeking a qualified, passionate Care Coordinator to join the team at our Fresh Meadows clinic in Queens. Responsibilities Care Coordinators are responsible for supporting patient care by making our patients feel welcomed and valuable while controlling the flow of appointments. They respond to all phone calls and emails within 2 hours, always within 24 hours. Care Coordinators guide patients through our out-of-network experience, greet all patients who enter the clinic with a smile, and execute 5-star customer service. They foster a true community environment for both patients and team members, collaborating to ensure everyone receives the care and support they need.
