Physician Assistant, Sr. Vascular Surgery (Stanyan and Hyde Hospitals)

Join our dynamic Vascular Surgery Team. This team provides advanced surgical and endovascular treatment of vascular disease. Our Vascular advanced practice providers manage patients across the continuum of care from new patient consultation to surgery to post operative follow up and surveillance management. The Vascular Surgery Physician Assistant serves as a high-level professional with clinical, surgical/procedural, administrative and educational responsibilities in the care of adult patients requiring vascular and endovascular surgery. Under the clinical supervision of the Vascular Surgeon and management of Stanyan administrative leadership in partnership with the Heart & Vascular Director and Center Director of Surgical Services, the Vascular Surgery Physician Assistant will have a critical role in the inpatient and outpatient settings with both team member and individual duties. These responsibilities will include preoperative and postoperative patient examination and assessment, diagnosis and planning; treatment implementation, coordination and continuity; performance of specific procedures; development of practice protocols and evaluation of quality of care; education of patients, families and other health care practitioners. The Physician Assistant will also be cross‑trained with colleagues on the General Surgical Service to provide mutual coverage during vacations, leaves, and periods of increased patient volume, as operational needs and staffing capacity permit. 30% Inpatient - Direct Patient Care Independently (APP encounter) or collaboratively (shared physician and APP encounter) evaluates and manages patients. Takes histories, performs physical examinations, responds to changes in patient status, performs consults, orders or furnishes medications/diagnostic tests/treatments/therapies/consultations/referrals/DME/home health services. Documents care in the electronic medical record per UCSF Rules and Regulations. Bills services in compliance with the APP Billing Policy 3.08.01. Responds to clinical documentation integrity inquiries within 3 days. Performs procedures granted via privileges/standardized procedures. Identifies patients with acute decompensation or failure, responds to hospital care plan, and reviews with physician. Initiates consultations as indicated. Documents independent assessment findings and recommends appropriate treatments. Participates in service rounds and patient evaluation and treatment planning. Reviews lab tests, x‑rays, and other diagnostics; initiates team plans. Evaluates complex cases and establishes individualized plans of care for disposition to a lower level. Interprets and acts on completed tests, procedures, and radiographic studies. Completes relevant EHR documentation, including medication orders, episodic care notes, and discharge summaries. Communicates findings, treatment options, and care plans to patients and families. May first‑assist in the operating room when privileged through OMAG after certification. Monitors and evaluates patients for discharge; sets parameters and guidelines for continuum of care with the multidisciplinary team; dictates discharge regimen. Discharges patients, prescribes/furnishes medications, orders DME, and recommends or refers for ongoing follow‑up; provides discharge teaching and completes summaries. 10% Inpatient - Care Coordination Coordinates care throughout the continuum including hospital admission, care coordination with staff and consulting services, and discharge planning. Coordinates episodic care and determines timing of routine clinic follow‑up. Participates in huddles with clinic/hospital staff, anticipating patients requiring close management or intervention. Consults with nursing staff on individualized nursing care plans. Independently provides case summaries for disability, insurance agencies, workers’ compensation, work releases, and medical supply agencies. Formulates and implements protocols to improve quality, efficiency, and reduce waste. 30% Outpatient - Independent Encounters Evaluates and manages patients in independent APP encounters, including telehealth. Provides care to new, follow‑up, acute, routine, and specialty patients. Takes histories, performs physical examinations, interprets diagnostic tests, formulates evidence‑based assessments and plans, responds to changes in patient status, and orders medications, diagnostics, treatments, therapies, consultations, referrals, DME, and home health services. Collaborates with attending physician as necessary. Documents care per UCSF Rules and Regulations. Bills services in compliance with APP Billing Policy 3.08.01. Performs procedures individually granted via standardized procedures/privileges. Identifies high‑risk groups and complex cases for review with attending physician; initiates consultations as appropriate. Evaluates complex cases, establishes individualized rehab plans, and facilitates occupational, physical therapy, and PM&R consultations. Interprets and acts on ordered tests, procedures, and radiographic studies in a timely manner. Completes relevant EHR documentation, including medication orders, episodic notes, and telephone encounters. 20% Outpatient - Shared Encounters with Physician Evaluates and manages patients in shared encounters with attending physician, including telehealth. Expands clinic access with a goal of doubling clinic volume by sharing responsibilities. Provides care to new, follow‑up, acute, routine, and specialty patients. Takes histories, performs physical examinations, interprets diagnostics, collaborates on assessments and plans, responds to changes in status, and orders medications, diagnostics, treatments, therapies, consultations, referrals, DME, and home health services. Documents care per UCSF Rules and Regulations. Bills services in compliance with APP Billing Policy 3.08.01. May first‑assist in the operating room when privileged through OMAG after certification. Identifies high‑risk groups and complex cases for review with attending physician; initiates consultations as appropriate. Evaluates complex cases, establishes individualized rehab plans, and facilitates occupational, physical therapy, and PM&R consultations. Interprets and acts on ordered tests, procedures, and radiographic studies promptly. Coordinates episodic care and determines timing of routine clinic follow‑up. Participates in huddles with clinic/hospital staff, anticipating patients requiring close management or intervention. Attends department quality improvement meetings and participates in patient evaluation and planning. Consults with nursing staff on individualized nursing care plans. Independently provides case summaries for disability, insurance agencies, workers’ compensation, work releases, and medical supply agencies. Required Qualifications Physician Assistant Clinical rotations with vascular surgery, cardiothoracic surgery, or general surgery within OR, ICU, and floor settings. Experience as first or second assist in the OR with management of vascular access for endovascular procedures. At least 6 months of experience working in an outpatient setting. Physician Assistant Senior Two years of inpatient vascular, cardiothoracic, or general surgery experience in the ICU and floor settings. Experience as first or second assist in the OR. One year of outpatient experience as an advanced practice provider in vascular surgery, cardiothoracic surgery, or general surgery (combined inpatient/outpatient minimum 2 years). Minimum License/Certification California Physician Assistant (PA) License without restriction. National Board Certification from the National Commission on Certification of Physician Assistants (NCCPA). California Driver License (if required). DEA Registration, schedules 2, 2N, 3, 3N, 4, and 5, registered to a California address. American Heart Association (AHA) Basic Life Support (BLS) certification. Additional required AHA certifications: ACLS. No additional required certifications (e.g., ENLS). Minimum Knowledge, Skills, and Abilities (KSA) Excellent verbal and written communication skills. Judgement skills to effectively meet patient needs. Self‑direction and organizational skills to function in an independent role. Ability to cope with personal stress experienced by health care providers and team members. Ability to meet minimum standards for hospital credentialing and billing as a billable provider. Submit health plan (Medicare, Medicaid, and commercial) enrollment by first clinical day. Adhere consistently to UCSF Medical Staff Bylaws, Rules and Regulations, Code of Conduct, PRIDE values, and applicable policies. Adhere to all UCSF policies regarding HIPAA, privacy, communication, documentation, encounter closure, and billing. Mandatory attendance at the Office of Advanced Practice Providers Orientation Sessions. Mandatory attendance at the APP Onboarding Committee APP Onboarding Series. Flexibility to work at all UCSF Health facilities/locations. Perform other duties as assigned. Preferred Qualifications Two or more years of PA experience managing vascular surgery patients in critical care/acute care/OR and outpatient environments. Two or more years of vascular surgery OR experience. Preferred Knowledge, Skills, and Abilities Experience providing independent consults as primary service provider. Possess the skills, knowledge, and abilities to care for vascular and general surgery patients in acute care, vascular surgeries, gastrointestinal, bariatric, colorectal, surgical oncology, hepatopancreatobiliary, and endocrine surgery categories.
