Key Responsibilities\n

As a Licensed Clinical Social Worker at Clarity Wellness Center, your daily responsibilities will encompass vital aspects of mental health counseling, including:

• Communicating effectively with physicians and other healthcare professionals to report patient progress, identify needs and concerns, and address any potential issues.

• Maintaining accurate and timely clinical records, including progress notes, in accordance with department policies.
\n
• Collaborating with a multidisciplinary team to develop and implement comprehensive treatment plans, including discharge strategies based on psychosocial assessments.
\n
• Providing individual and group counseling sessions, utilizing crisis intervention techniques to address emotional and behavioral challenges.
\n
• Facilitating discharge planning and care coordination by connecting clients to appropriate community resources and services.
\n
• Consulting with clients' families or support systems to ensure comprehensive treatment and effective discharge planning.

Patient Population / Client Focus\n

You will primarily work with a diverse patient population, including adults experiencing a range of mental health challenges such as anxiety, depression, trauma, and adjustment disorders. Your role will be pivotal in helping these individuals navigate their personal struggles and achieve holistic well-being, ultimately enhancing their quality of life.

Qualifications\n

The ideal candidate will have:

• A Master’s degree in Social Work or Counseling from an accredited program.

• A valid California LCSW license.
