Clinical Manager, Home Health & Hospice RN Wesley At-Home Pay Class: Full-Time Shift: Day Shift Work Availability: Available Mon-Fri We are seeking a dedicated licensed Clinical Manager to oversee and coordinate the daily clinical operations of our Home Health & Hospice programs in Washington State. The licensed Clinical Manager is responsible for ensuring high‑quality patient care, compliance with state and federal regulations, and efficient management of clinical staff. This position plays a key role in maintaining DOH (Washington State Department of Health) compliance, Medicare/Medicaid regulations, and accreditation standards while fostering a culture of excellence, teamwork, and compassionate care. Clinical Oversight & Quality Assurance Ensure all patient care services adhere to Washington State Department of Health (DOH) home care and hospice licensing regulations. Maintain compliance with Medicare/Medicaid Conditions of Participation and accreditation standards (Joint Commission, CHAP, or ACHC). Oversee patient assessments, care planning, and documentation to ensure accuracy, quality, and compliance. Implement and monitor quality assurance and performance improvement programs to enhance patient outcomes. Stay current with Washington State’s opioid prescribing laws, advanced directives regulations, and end‑of‑life care policies. Supervise and support RNs, LPNs, therapists, social workers, chaplains, bereavement staff, volunteers, and home health and hospice aides. Conduct staff training and competency evaluations per Washington State DOH guidelines. Lead case conferences and interdisciplinary team meetings to promote continuity of care. Ensure compliance with Washington’s continuing education requirements for nursing professionals. Build a culture of belonging through active use of the company’s recognition and reward platform. Create an engaged and high‑performing team through adoption and visual support of the company’s people strategies. Regulatory Compliance & Documentation Ensure timely, accurate, and complete documentation in Electronic Health Records in compliance with Washington State home health & hospice documentation standards. Prepare for state DOH inspections, Medicare audits, and accreditation surveys. Maintain compliance with Washington’s POLST and Advance Directive requirements. Address and resolve patient grievances and complaints in accordance with Washington State Patient Rights regulations. Manage staff scheduling and resource allocation to optimize care delivery while maintaining cost efficiency. Work with leadership to ensure financial sustainability and proper utilization of Medicare and Medicaid reimbursements under Washington State guidelines. Assist in the development of policies and procedures specific to home health and hospice services in Washington. Patient & Family Advocacy Serve as a liaison between patients, families, caregivers, and community resources to ensure comprehensive support. Educate patients and families on Washington’s Death with Dignity Act, hospice eligibility criteria, and palliative care options. Promote community outreach, patient education, and end‑of‑life planning initiatives. Leverage Lean thinking and methodology to develop contingency plans and staff competency with service recovery. Requirements and Qualifications Active, unrestricted RN license in Washington State (WA DOH licensed) or out‑of‑state MSL. Follow the Washington Nurse Practice Act. If MSL, complete Suicide Prevention Training in advance of hire. If MSL, complete Washington State Board of Nursing MSL demographic survey in advance of hire. Bachelor’s degree in Nursing (BSN) required; Master’s degree (MSN, Healthcare Administration, or related field) preferred. Minimum of 2‑4 years of experience in home health, hospice, or a related healthcare field. Strong knowledge of Washington State DOH home health and hospice regulations, Medicare/Medicaid requirements, and accreditation standards. Leadership experience with staff supervision, team management, and performance evaluations. Proficiency in Electronic Health Records and case management software. Strong organizational, problem‑solving, and decision‑making skills. Excellent communication and interpersonal skills with a focus on patient‑centered care. Experience with survey readiness, quality assurance, and performance improvement programs. Knowledge of Washington Medicaid Home and Community‑Based Services waiver programs. Certification in Hospice & Palliative Nursing (CHPN), OASIS, or other relevant credentials. Bilingual skills; preferred but not required. Driving is an essential function; a valid driver’s license is required. Physical, Environmental, and Mental Requirements Ability to give direct patient care, including lifting, transferring, positioning, and assistance of residents with walking or in wheelchairs. Able to lift 40 lbs on a regular basis. Ability to use a laptop, tablet, or touch screen to document resident records. Apply physical and muscle strength to lift, push, pull or carry objects. Use lower back and abdominal muscles to support part of the body repeatedly and continuously without fatigue. Ability to listen to and comprehend information and ideas conveyed through conversation, email, and text. Maintain focus and concentration.
