Position Summary

We are seeking a compassionate and dedicated Speech Language Pathologist (SLP) to provide high-quality speech and language therapy services in patients' homes. This part-time opportunity offers a flexible weekday schedule and the chance to work with a diverse patient population in their home environment.

Key Responsibilities

• Evaluate, diagnose, and treat speech, language, communication, cognitive, voice, and swallowing disorders.
• Conduct comprehensive assessments and develop individualized treatment plans based on each patient's needs, goals, and physician orders.
• Provide skilled speech therapy services in patients' homes while adhering to evidence-based clinical practices.
• Monitor patient progress, reassess functional outcomes, and modify treatment plans as needed.
• Educate patients, families, and caregivers on therapeutic techniques, communication strategies, and home exercise programs.
• Collaborate with physicians, nurses, physical therapists, occupational therapists, case managers, and other healthcare professionals to coordinate patient care.
• Maintain accurate, timely, and compliant clinical documentation, including evaluations, progress notes, treatment records, and discharge summaries.
• Ensure compliance with all applicable regulatory and organizational standards.
• Participate in interdisciplinary care planning and contribute to positive patient outcomes.
• Promote patient safety, independence, and quality of life throughout the course of treatment.

Qualifications

• Master's degree in Speech-Language Pathology from an accredited program.
• Active Speech Language Pathologist license, or eligibility for licensure in the state of practice.
• Minimum of one (1) year of experience as a Speech Language Pathologist.
• Additional state-required certifications or credentials as applicable (assistance may be provided to obtain required certifications).
• Current CPR/BLS certification, if required.
• Valid driver's license, reliable transportation, and automobile insurance for travel between patient homes.

Preferred Qualifications

• Previous home health experience.
• Experience working with adult and geriatric populations.
• Knowledge of dysphagia management, cognitive rehabilitation, and communication disorders.
• Familiarity with EMR/EHR documentation systems.

Work Schedule

Part-time (20-25 hours per week).

Monday through Friday with flexible scheduling.

No routine on-call or weekend responsibilities.
