Job Details:

• Title: Clinical Pharmacist – Hospital
• Location: Fort Walton Beach, FL 32547
• Job Type: Full-Time
• Shift: Days | 7:00 AM – 5:30 PM
• Schedule: 4 × 10-hour shifts
• Weekends: Every 3rd weekend
• Holiday Rotation: 1 summer holiday (Memorial Day, Independence Day, or Labor Day) and 1 winter holiday (Thanksgiving, Christmas, or New Year's)
• Salary: $52.00 - $64.00/hour (Based on experience) + Benefits
• No Sign-On Bonus
• Relocation Assistance: Case-by-case

Shift Differentials:

• 2nd Shift: +$2.00/hour
• 3rd Shift: +$4.25/hour
• Extra Shift Premium: +$3.00/hour

Position Overview

We are seeking a Clinical Pharmacist to join our hospital pharmacy team. This is an excellent opportunity for a pharmacist with hospital experience or a recently completed PGY1 residency who is looking for a long-term opportunity with a collaborative team and opportunities for professional growth.

-> Responsibilities :

• Interpret medication orders and accurately enter or verify them in the pharmacy information system.
• Compound and dispense medications accurately and efficiently.
• Monitor medication therapy for contraindications, drug-drug interactions, drug-food interactions, allergies, and appropriate medication selection and dosing.
• Support the hospital formulary and minimize nonformulary medication procurement through appropriate therapeutic interchange and rational drug therapy selection.
• Provide timely and accurate medication information to healthcare professionals and patients.
• Participate in clinical pharmacy services, including:
• • Drug information consultations
  • Nutrition support
  • Pharmacokinetic monitoring
  • Anticoagulation monitoring
  • Pain management
  • IV-to-PO conversion
  • Renal dose adjustments
  • Other clinical programs established by Pharmacy Leadership
• Accurately document clinical activities and pharmacist interventions.
• Complete medication error and adverse drug reaction reports.
• Support and supervise pharmacy operations while contributing to the success of the entire pharmacy team.
• Collaborate effectively with physicians, nurses, pharmacy staff, and other members of the healthcare team.

-> Required Qualifications

• Bachelor's degree or PharmD from an ACPE-accredited School of Pharmacy .
• Active pharmacist license in good standing with the Florida State Board of Pharmacy .
• 1+ year of hospital pharmacist experience preferred.
• Candidates who have successfully completed a PGY1 residency will also be considered.
• Strong clinical judgment, communication, teamwork, and organizational skills.
• Commitment to providing safe, high-quality, patient-centered medication care.
