What You\u2019ll Do

• Serve as the primary clinical partner for patients and families, delivering individualized, one\u2011on\u2011one nursing care in the home that promotes recovery, safety, and independence.
• Conduct comprehensive in\u2011home assessments to evaluate physical, emotional, and environmental needs, and translate those findings into thoughtful, patient\u2011centered care plans.
• Provide skilled nursing interventions\u2014including wound care, infusions, catheter care, medication management, post\u2011operative care, and chronic disease management\u2014tailored to each patient\u2019s goals and condition.
• Educate and empower patients and caregivers through clear instruction, coaching, and support, helping them confidently manage symptoms, medications, and daily care at home.
• Monitor patient progress closely, identify changes in condition early, and communicate effectively with physicians and the interdisciplinary care team to adjust plans and prevent avoidable hospitalizations.
• Coordinate care across providers and services, ensuring continuity, clarity, and high\u2011quality outcomes throughout the patient\u2019s home health journey.
• Complete timely, accurate documentation that reflects the full clinical picture and supports quality, compliance, and measurable outcomes.

The Nurse We\u2019re Seeking

• Holds an active Registered Nurse (RN) license in the state of practice with at least one (1) year of clinical nursing experience
• Is patient\u2011focused, organized, and motivated by delivering high-quality care
• Has a current driver\u2019s license, reliable transportation, and auto insurance
• Is CPR certified (or able to obtain certification)
