Responsibilities
- Conduct initial clinical assessments to identify patient needs and determine appropriateness for physical therapy; document Plan of Care (POC), progress notes, and other clinical records, revising the POC as evaluation and assessment data evolve; collaborate with the physician and interdisciplinary team.
- Plan and provide treatment for neuromuscular, neurological, and orthopedic disorders based on medical referral and therapy assessments; adapt interventions as patients progress.
- Instruct patients and families on transfer techniques, use of equipment (walkers, wheelchairs, canes, braces, prosthetic devices), and appropriate home modification or equipment needs; order and adapt rehabilitative equipment as required.
- Deliver patient/family education on therapeutic procedures, safety, and home-care strategies, tailoring to age, condition, culture, and learning readiness.
- Supervise and coordinate care with other team members when applicable, while maintaining infection control practices and universal precautions.

Requirements
- Current State license as a Physical Therapist
- Proof of current CPR certification
- Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program
- 1–2 years of experience as a Physical Therapist in an acute care or rehabilitation setting
- Previous home care experience preferred
- Current driver’s license with reliable transportation and insurance; ability to travel locally
