PATIENT NAVIGATOR

Patient Navigator Callen-Lorde Community Health Center is seeking a Patient Navigator to join our dedicated team and play a vital role in supporting the needs of our patients living with HIV. The role places a strong emphasis on prevention and patient education, including HIV prevention strategies such as PrEP and PEP, long‑acting injectable (LAI) treatments, and broader preventive care services. Experience or familiarity with sexual health education and adult patient care is highly valued. As a Patient Navigator, your responsibilities will include: Act as a patient advocate, ensuring patients' biopsychosocial needs are identified, addressed, and connected to appropriate services. Provide field‑based case management and adherence support services to HIV‑positive patients. Work collaboratively with care coordinators, medical providers, and behavioral health professionals to facilitate patient retention and adherence to primary medical care. Assist patients in navigating the healthcare system, overcoming barriers to care, and connecting them with essential resources. Advocate for patients' rights and ensure they receive culturally competent care. Document patient interactions and progress notes to ensure continuity of care. Contribute to a team‑oriented environment, fostering a supportive and inclusive space for patients. This is a full‑time position with evening hours. The schedule is structured to support operations across our Bronx, Brooklyn, and Chelsea sites. Please note that this position is grant funded. Qualifications High school diploma or GED required. CPR certification required. Experience in social services, case management and/or entitlements work. Strong computer competency, including knowledge of Electronic Medical Records, Microsoft Office Programs, and internet research savvy. Strong customer service orientation with an understanding of patient‑led treatment. Experience working with individuals with a spectrum of chronic illness conditions preferred. Desired Characteristics, Skills Ability to work independently yet be team focused. Ability to remain calm, focused, and helpful under stress. Knowledge of Microsoft Office programs and utilization of these tools for scheduling and communication. Strong communication and interpersonal skills.
