Behavioral Health Patient Navigator

LaSante Health Center is seeking a compassionate and organized Behavioral Health Case Manager to support patients receiving mental health services within our outpatient behavioral health clinic in Brooklyn. This role focuses on helping patients navigate care, addressing barriers to treatment, assisting with coordination of services, and maintaining engagement in behavioral health treatment. The ideal candidate is personable, proactive, and comfortable working with underserved populations in a fast‑paced clinic environment. Responsibilities Assess patients for social, behavioral health, and practical support needs Assist patients with accessing community resources and services Help coordinate care between providers and outside organizations Conduct outreach calls to schedule, confirm, and follow up on appointments Support patient engagement and retention in treatment Document interactions and patient needs within the EMR Work collaboratively with therapists, psychiatrists, medical providers, and administrative staff Assist with referrals, transportation coordination, insurance issues, and other patient support needs Qualifications High school diploma or equivalent required Associate’s or bachelor’s degree preferred but not required Experience in healthcare, behavioral health, community outreach, care coordination, or customer service strongly preferred Strong communication and organizational skills Comfortable speaking with patients over the phone and in person Bilingual candidates encouraged to apply Experience working with Medicaid or underserved populations is a plus What We Offer Competitive compensation Supportive team environment Opportunity to make a meaningful impact in an underserved community Growth opportunities within a rapidly expanding behavioral health program LaSante Health Center is committed to improving access to quality behavioral healthcare for the communities we serve. #J-18808-Ljbffr LaSante Health Center
