Job Description
Job Description
Family Nurse Practitioner / Adult-Gerontology Nurse Practitioner
Overview & Highlights- Location: Primarily Lake Havasu City, with some Kingman and Bullhead City coverage in the future if needed
- Schedule: Full-time with long-term scheduling flexibility
- Compensation: $110,000 to $150,000 per year base, depending on experience, with huge upside potential to earn $200,000+ per year
- Setting: Community-based geriatric care within senior living communities
- Relocation / Sign-On Bonus: Assistance may be considered for the right candidate
- Training: Extensive hands-on onboarding, clinical mentorship, documentation support, and ongoing coaching
- Growth: Potential opportunities in education, training, research, leadership, and future university collaborations
This is more than a traditional Nurse Practitioner position. It is an opportunity to become an early clinical leader in a growing geriatric healthcare organization addressing a significant shortage of care throughout Lake Havasu and the surrounding region.
Many older adults in this community have difficulty accessing consistent primary care. Some must travel hours for appointments, while others lack a dependable provider who understands their medical history, living environment, family dynamics, and long-term goals.
You will help change that.
As an FNP or AGNP, you will provide comprehensive primary care to older adults living in assisted living, memory care, and independent living communities. You will build lasting relationships with patients, families, facility leaders, caregivers, and community partners while helping establish a higher standard of accessible, responsive geriatric care.
The organization’s care philosophy is informed by person-centered and Montessori-based principles. Patients are treated as individuals with purpose, preferences, abilities, and meaningful lives, not simply as diagnoses or a collection of medical conditions.
Training and Ramp-UpYou will receive a level of hands-on support that is uncommon in highly autonomous provider roles.
Your initial training is expected to include:
- Direct support from the organization’s owners and clinical leadership
- Clinical guidance and physician collaboration
- Training with an experienced Nurse Practitioner consultant
- Dedicated Nurse Practitioner coaching during the first several months
- Education regarding geriatric care delivery in senior living communities
- Documentation, billing, coding, credentialing, and compliance training
- Coaching on communicating with patients, families, facility leaders, and caregivers
- Support developing an efficient and sustainable daily workflow
- Ongoing feedback focused on identifying your strengths and accelerating your professional growth
The organization’s leadership team will be highly involved during the launch. You will not be expected to build the practice alone or figure everything out without support.
What the First Year May Look LikeFirst 30 Days
The initial month will focus heavily on training, onboarding, credentialing, documentation, billing processes, clinical expectations, community introductions, and preparation to begin building the patient panel.
Months Two Through Six
You will progressively increase patient visits and community rounding across assisted living, memory care, and independent living communities.
A typical day may eventually include approximately 7 to 12 patient interactions, depending on patient complexity, travel, telehealth visits, care coordination, and administrative responsibilities.
During this launch period, patient and community needs may require availability across six days and participation in an on-call schedule. Responsiveness to urgent patient, family, and facility concerns will be essential.
After the Initial Ramp-Up
Leadership will evaluate patient volume, workload, scheduling, and the provider’s professional goals. The intention is to create a sustainable practice model that offers meaningful autonomy, flexibility, and room for continued growth.
A Day in the LifeYour day will be varied and highly relationship-driven. Rather than practicing from one traditional clinic location, you will travel among senior living communities to care for patients where they live.
You may begin the day rounding at a memory care community, meet with a family concerned about a change in a patient’s behavior, coordinate with facility staff regarding a medication adjustment, complete follow-up visits at an assisted living community, and conduct telehealth or care-coordination work later in the day.
You will manage the full spectrum of geriatric primary care, including chronic disease, preventive care, acute concerns, medication management, cognitive decline, changes in function, and transitions between healthcare settings.
Many patients will have multiple chronic conditions and complex medication regimens. Strong clinical judgment is important, but success also requires patience, time management, emotional intelligence, thorough documentation, and the ability to communicate clearly with families and caregivers.
The Personality That Will Thrive HereThe organization describes its strongest team members through three qualities:
Competition. Accessibility. Communication.
Competition
This does not mean competing against coworkers. It means having an internal drive to pursue excellence and win for the patient.
The right provider wants to raise the standard of care, advocate fiercely for older adults, follow through on commitments, and continually become better at their work. Clinical skills can be developed through training, but the motivation to excel must come from within.
Accessibility
Patients, families, facilities, and team members need a provider who is responsive and dependable. That means returning calls, addressing concerns promptly, following through, and recognizing when a delayed response could negatively affect a patient.
Healthy professional boundaries remain important, but this is not a role for someone who routinely becomes unreachable when the workday ends, particularly during the initial practice-building phase.
Communication
Excellent care depends on communication with more than the patient. You must be able to work effectively with families, caregivers, facility administrators, nurses, therapists, specialists, billing staff, and clinical leadership.
Some conversations will involve anxious family members, behavioral changes, cognitive decline, goals of care, or emotionally difficult decisions. The right provider can listen compassionately, explain clinical information clearly, establish appropriate boundaries, and keep everyone moving toward the patient’s best interests.
Why This Role Stands Out- Make an immediate community impact: Help bring dependable primary care to an underserved population of older adults.
- Build something meaningful: Play an important role in launching and shaping a growing regional practice.
- High clinical autonomy: Manage your patient panel and make independent clinical decisions with minimal micromanagement.
- Hands-on leadership support: Work closely with experienced healthcare operators, clinical leadership, and an NP coach.
- Meaningful continuity of care: Develop lasting relationships with patients and families rather than providing isolated episodes of care.
- Substantial professional growth: Strengthen your skills in geriatrics, documentation, care coordination, practice development, leadership, and healthcare operations.
- Significant earning potential: Providers interested in building a larger patient panel may have the opportunity to earn $200,000 or more annually.
- Personalized career development: Leadership takes an individualized approach to helping team members pursue their professional, educational, and financial goals.
- Future educational opportunities: As university relationships and a planned dementia education initiative develop, there may be opportunities to contribute to training, research, curriculum development, or guest instruction.
- Long-term flexibility: The position is designed to become more flexible and sustainable as the initial launch phase is completed.
- A genuine passion for serving older adults
- An extra-mile mentality and strong commitment to patient advocacy
- A competitive internal drive to pursue excellence
- High personal accountability and dependable follow-through
- Strong clinical judgment and comfort with medically complex patients
- Excellent time-management and organizational skills
- Prompt, professional communication
- Comfort interacting with patients, families, caregivers, and facility leaders
- The emotional intelligence to navigate difficult or sensitive conversations
- Thorough, individualized, and timely clinical documentation
- Interest in understanding the operational and business side of healthcare
- Ability to work independently while knowing when to seek collaboration
- Comfort working in a growing organization where processes will continue to evolve
- Coachability, curiosity, and a genuine desire to improve
- Provide comprehensive primary care to older adults living in memory care, assisted living, and independent living communities
- Evaluate and manage chronic diseases, acute concerns, cognitive changes, and medically complex geriatric conditions
- Conduct preventive care, routine assessments, and follow-up visits
- Develop individualized care plans focused on health, function, independence, dignity, and quality of life
- Manage medications and reduce unnecessary polypharmacy when clinically appropriate
- Identify changes in patient condition and respond before they become larger medical problems
- Communicate promptly with patients, families, caregivers, and facility leadership
- Collaborate with physicians, nurses, therapists, specialists, hospice providers, home health organizations, and other community partners
- Coordinate care across senior living communities, hospitals, specialists, and other healthcare settings
- Educate patients and families regarding diagnoses, medications, treatment plans, and goals of care
- Maintain accurate, individualized, complete, and timely documentation
- Support appropriate billing, coding, compliance, and quality standards through thorough clinical notes
- Participate in the practice’s on-call coverage model
- Help develop efficient workflows and contribute ideas as the organization grows
- Exercise independent clinical judgment while seeking physician collaboration when appropriate
- Represent the organization professionally within senior living communities and throughout the Lake Havasu region
- Licensed Nurse Practitioner with FNP or AGNP preparation

- DEA registration or ability to obtain
- Strong clinical decision-making and critical-thinking skills
- Excellent interpersonal and communication skills
- Ability to practice independently and collaboratively
- Strong documentation and time-management habits
- Reliable transportation and willingness to travel throughout the local service area
The organization is highly experienced in coaching and developing talented professionals, including those entering a new specialty or stage of their careers.
A new graduate can be successful here if they demonstrate maturity, initiative, strong communication, confidence balanced with humility, a genuine passion for geriatrics, and an exceptional willingness to learn.
