Home Care Intake & Referrals Coordinator
Catholic Health Services
Catholic Health is seeking a Home Care Intake Specialist to support the admission process by coordinating referrals with Home Care Transition RNs and entering data into the EMR. You will be a central contact for hospitals, physician offices, and referral sources, ensuring timely Start of Care and accurate referral processing. The ideal candidate has a Bachelor's degree and healthcare experience, with strong organizational and communication skills and proficiency in Microsoft Office and EMR #J-18808-Ljbffr Catholic Health Services
