JOB SUMMARY

The Assistant Director of Nursing (ADON) supports the Director of Nursing in the daily oversight and coordination of clinical operations for home health and specialty infusion services. This role is designed as a succession pathway to the Director of Nursing position, providing progressive leadership responsibility and exposure to all aspects of clinical, regulatory, and administrative functions. The ADON assists in ensuring the delivery of high-quality, safe, and compliant patient care in accordance with federal, state, and agency standards. The ADON actively participates in clinical supervision, staff development, quality improvement, and operational workflows, while developing the competencies necessary to transition into the DON role.

QUALIFICATIONS

• Graduate of an accredited school of professional nursing; currently licensed as a Registered Nurse in the State of Illinois, in good standing.

• Minimum of 2 years of home health experience required.

• Demonstrated leadership potential with strong organizational, communication, and problem-solving skills.

• Working knowledge of home health regulations, OASIS, and accreditation standards.

• Proficiency or willingness to learn EMR systems (Axxess), workflow tools (ClickUp), and compliance training platforms (Relias).

• Ability to exercise sound clinical judgment and foster a collaborative team environment.

ESSENTIAL RESPONSIBILITIES

• Clinical Oversight & Support

  • Assist the DON in ensuring compliance with all applicable laws, regulations, and agency policies.

  • Support the development and enforcement of clinical standards for safe, effective patient care.

  • Participate in clinical record review, chart audits, and documentation compliance (including OASIS).

  • Assist in the development, review, and oversight of individualized patient care plans.

  • Support coordination of care with physicians, therapists, and interdisciplinary team members.

• Staff Supervision & Development

  • Assist with supervision, training, and competency validation of clinical staff.

  • Participate in onboarding and orientation of new clinicians, including EMR and workflow training.

  • Support performance evaluations, coaching, and corrective action processes.

  • Promote staff engagement, accountability, and adherence to best practices.

• Operations & Workflow Management

  • Assist with staffing, scheduling, and visit assignment coordination.

  • Support intake, referral, and admission processes to ensure timely service delivery.

  • Utilize systems such as Axxess and ClickUp to monitor workflows, task completion, and compliance.

  • Participate in on-call rotation and provide clinical support as needed.

• Quality Assurance & Compliance

  • Participate in Quality Assurance and Performance Improvement (QAPI) activities.

  • Assist in monitoring incident reports, infection control logs, and risk management processes.

  • Support accreditation readiness and regulatory survey preparation.

  • Ensure timely follow-up on clinical issues, complaints, and variances.

• Leadership Development & Administrative Support

  • Attend and actively participate in IDT and management meetings.

  • Assist in policy and procedure review and updates.

  • Collaborate with the DON and Administrator on strategic and operational initiatives.

  • Serve as acting DON in delegated situations or in the absence of the DON.

  • Engage in ongoing leadership development in preparation for DON transition.

• Additional Duties

  • Perform field visits as needed to support patient care and staff competency.

  • Maintain strict confidentiality of patient and agency information.

  • Fulfill additional duties as assigned.

WORKING CONDITIONS

• Work is performed in an office environment with required travel to patient homes, as needed.

• Exposure to high-risk clinical situations.

• Must be able to lift up to 50 lbs and perform clinical tasks as needed.

• On-call responsibilities as assigned.
