Key Responsibilities\n

• Conduct thorough psychosocial assessments to ascertain patients' emotional, social, and financial challenges, and work collaboratively with medical teams to create individualized care plans.

• Facilitate both individual and group counseling sessions that focus on adjustment issues, grief processing, and coping strategies tailored to each patient’s specific circumstances.

• Coordinate with community organizations and resources to arrange post-discharge services, ensuring a seamless transition back into the community and continuity of care.

• Provide crisis intervention services to patients and families in need, addressing immediate psychological distress and offering emotional support.

• Develop, implement, and continually revise discharge plans, connecting patients with resources that enhance their long-term recovery success.

• Maintain detailed, accurate, and timely documentation to fulfill legal and regulatory requirements while adhering to our hospital’s policies.

Qualifications\n
