Position Summary

The Nursing Home Maintenance Representative is responsible for maintaining, strengthening, and expanding relationships with existing nursing home, skilled nursing facility, rehabilitation, and other healthcare referral accounts.

This position serves as an ongoing point of contact for referral partners, helping ensure strong communication, consistent follow-up, and continued referral activity. The ideal candidate is relationship-driven, highly responsive, organized, and comfortable communicating regularly with healthcare professionals and facility leadership.

Key Responsibilities

• Maintain and strengthen relationships with existing nursing homes, skilled nursing facilities, rehabilitation centers, and other assigned referral accounts.
• Maintain regular contact with administrators, social workers, discharge planners, case managers, and other referral partners.
• Conduct ongoing outreach and follow-up with assigned accounts to maintain visibility and strengthen relationships.
• Identify opportunities to increase referrals within existing accounts.
• Educate referral partners regarding available home care services, programs, and capabilities.
• Respond promptly to questions, concerns, and referral needs from facility partners.
• Coordinate with intake, clinical, marketing, and operations teams to help facilitate referrals and admissions.
• Monitor referral activity within assigned accounts and identify changes, opportunities, or potential concerns.
• Maintain accurate documentation of account contacts, visits, referrals, and follow-up activity.
• Re-engage referral sources when activity declines and work to strengthen underperforming accounts.
• Represent the organization professionally during facility visits, meetings, and healthcare-related events.
• Meet established account-maintenance and referral-development expectations.
