Job Description

• Implement therapeutic interventions aimed at enhancing mobility, strength, balance, and overall function
• Collaborate seamlessly with nurses, physicians, case managers, and rehab professionals to optimize patient recovery
• Educate patients and their families on therapeutic objectives, discharge processes, and home exercise regimens
• Maintain accurate documentation of assessments, treatments, and patient progress, adhering to facility and regulatory standards
• Engage actively in care team meetings to ensure comprehensive and coordinated patient care

Education

• Doctorate in Physical Therapy (DPT) or equivalent from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification preferred
• Specialized certifications (such as neurological or geriatric) are advantageous but not mandatory

Skills

• Preferred prior experience in a similar role
