Medical Social Worker

Job Description ***Bilingual in Spanish***Medical Social Worker (MSW) – Long-Term Care Facility We are seeking a dependable and compassionate Medical Social Worker (MSW) to provide social services for residents of a 16-bed Congregate Living Health Facility (CLHF). Our residents are primarily long-term residents with complex medical and psychosocial needs. Responsibilities: Complete initial and new admission social service assessments within 5 days of admission. Complete required quarterly assessments for long-term residents. Complete assessments, progress notes, and other required documentation using PointClickCare (PCC) electronic health record software. Identify residents' psychosocial needs and provide appropriate interventions and follow-up. Participate in care planning and interdisciplinary coordination as needed. Communicate with residents, families, nursing staff, physicians, and other members of the interdisciplinary team. Maintain timely and accurate documentation in accordance with facility policies and regulatory requirements. Provide resources, referrals, and support to residents and families as appropriate. Be available by phone as needed for resident-related social service concerns. Qualifications: Master's degree in Social Work (MSW) from an accredited program. Experience in long-term care, skilled nursing, congregate living, or another healthcare setting preferred. Experience with PointClickCare (PCC) preferred; training can be provided. Strong assessment, documentation, communication, and organizational skills. Ability to work independently while collaborating with the interdisciplinary healthcare team. Dependable and responsive to resident and facility needs. This position is ideal for an MSW interested in working with a small, stable long-term care population and maintaining continuity of care with residents.
