As an integral member of our nursing team, we are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) or Registered Nurse (RN) to serve as our MDS Coordinator. In this critical role, MDS Coordinator will work closely with our nursing staff, healthcare providers, and residents to ensure the highest quality of care and support. Responsibilities: Coordinate the maintenance and completion of Minimum Data Set (MDS) assessments, care plans, and documentation for our residents Collaborate with nursing staff, healthcare providers, and residents to identify and address changes in resident conditions and needs Develop and implement individualized care plans that meet the unique needs of our residents Ensure accurate and timely completion of MDS assessments and care plans Maintain accurate and up-to-date records and reports Participate in quality improvement initiatives to enhance the overall quality of care and services Requirements: Current Licensed Practical Nurse (LPN) or Registered Nurse (RN) licensure in the state of Missouri Experience in a long-term care or senior living setting preferred Strong clinical knowledge and skills in geriatric care Excellent communication, interpersonal, and organizational skills Ability to work effectively in a fast-paced environment and prioritize tasks and responsibilities What We Offer: Advanced Pay (hourly employees) 401(k), Holiday Pay & Paid Time Off (PTO) Dental, Health, Vision, Life insurance & more Competitive Wages Advancement Opportunities Exclusive Aspire Senior Living Perks $4,000 MDS Full-Time Sign-on Bonus available! #J-18808-Ljbffr Heritage Senior Living
