Hospice Field Admissions RN - Sign-On Bonus Eligible

MJHS Hospice is seeking an experienced RN for field admissions to assess potential patients in homes or facilities and to admit them to our hospice program. You will perform physical and psychosocial assessments and help craft individualized care plans and goals. The role requires an active NYS RN license, a valid NYS driver’s license, and 2+ years in med/surg, community health, or oncology settings. A $15,000 sign-on bonus is offered. #J-18808-Ljbffr MJHS
