Hospice Field Admissions RN — Impactful Home-Based Admit

MJHS is seeking a Hospice Field Admission Nurse (RN) to perform initial physical and psychosocial assessments of potential hospice patients. You will develop individualized plans of care and goals, making recommendations based on each patient’s needs. Assessments may occur in private homes, hospitals or nursing facilities, and you will admit eligible patients onto the Hospice program following NHPCO standards. #J-18808-Ljbffr MJHS
